Step 3: Sign it (Applicant’s / Administrator’s Signature on the application form CANNOT be Electronic and MUST be ID Verifiable).
Step 4: Fax it to 713 776 2812 or email to sandra@cradlesolution.com or sales@cradlesolution.com
Step 5: Assign eSignature Role to applicant in CradleMRx.
